How to Register • Fill out the Education for Life registration form – to download click here• Email or fax the completed form to EFL at info@educationforlife.org or (503) 693-3216 o If you are signing up for a renewal course you will also need to send: • a copy of the front and back of your current provider card OR • if you took your last class with EFL you can list your last class date and location and we can verify it for you. • Once you have submitted your form, and supporting documents, please call our office to verify that we have received them and to provide a credit card for payment or deposit. • Once we have received your registration form, any required provider cards and your payment you will be added to the class and will receive a welcome email with in one week, if you do not receive the confirmation email please contact our office. Your registation cannot be competed until we have received all of the required information and payment. All registration information must reach EFL 7 days prior to class date. Late Registration: Registrations received after the 7 day cut off will be considered based on available space in the class and will be charged a $50 late fee payable by credit card or money order. Refund Policy: Cancellations received 7 days prior to class will receive a refund less a $50 cancellation fee. No refunds after the 7-day cut off. NO EXCEPTIONS. Refund for textbooks will be issued if the textbook is returned to our office unopened with in 30 days of purchase. Opened textbooks can not be returned.
